WE ACCEPT MOST INSURANCE PLANS
Blue Cross Blue Shield - Preferred provider - Often their plans are a 20% copay after the deductible is met. Federal plans and a few others often have a set copay. Call for details on how you are covered through BCBS.
United Health Care (Optum) - We are IN-Network providers with United Health Care/OptumHealth. Most United plans that we see have set copays. Call for details on how you are covered through UHC.
Medicare - Participating provider - Medicare pays 80% of the fee for spinal adjustment. The patient only pays 20% of the spinal adjustment fee up front which is usually between $5-$10. Unfortunately, medicare doesn't pay for exams, therapies or rehab. Call for details of how this works. We do our best to make sure that your health care costs are a good value.
How to become a patient at Millbrook Family Chiropractic
We would love for you to fill out your new patient forms online and send them electronically. The tab at the top of this page on the far right labeled "New Patient Health History Form" goes line by line and is easy and quick for you and for us.
If you prefer to do this stuff in writing use the tab toward the left side labeled "New Patient Center" then select "Online Forms". Scroll down a bit and you will see the heading New Patient Health History Form and a spot to click for download. Just download the forms, print, fill them out and you've saved probably 15 minutes of wait time in our front office!
